The standard form for a Consumer who wants to notify withdrawal from an Etapai distance contract.
Provide only the information needed to identify your contract. If electronic submission is unavailable, send the withdrawal statement to info@etapai.lt. The Terms of Service explain the conditions and 14-day period.
